REGISTRATION

Registration is held each February for those wishing to attend the Academy the following fall. Registration for current students wishing to return for the next academic year is held a week prior to registration for new students.

Required documents:
*  copy of birth certificate
   copy of baptismal certificate (Catholics only)
   social security number
**copy of most recent report card
**copy of most recent standardized test results
   copy of testing for exceptional students
          services (gifted, learning disabilities,
          speech, etc.)

*Kindergarten and first grade applicants must supply an original copy of a birth certificate to ensure the child meets the age requirements established by Florida law.

**Not required for kindergarten

Family Registration Form - Kindergarten
must be completed by families with a child or children entering kindergarten in the 2010-2011 school year

Family Registration Form 1st - 8th Grade - New Families
must be completed by NEW families with a child or children entering first through eighth grades in the 2010-2011 school year

Student Registration Form
must be completed for each student

Authorization Form
must be completed for each student

Request for Information Prior To Acceptance
must be completed by student's current school for all new students entering first through eighth grades in the 2010-2011 school year

Permission From Pastor Form
must be completed for each family

Verification of Involvement in Parish Ministry
must be completed for each family

Financial Aid Application
must be completed for each family applying for financial aid. Completed form must be postmarked no later than
March 15, 2010.
